Specimen Requirements

Blood
Draw 1 full, lavender-top (EDTA) tube, and send EDTA whole blood at ambient temperature only.
Note:  1. Label tube with patient’s name (first and last), date of birth, date and actual time of draw, and label specimen as peripheral blood.
2. Complete a “Christian Hospital Hematopathology-Bone Marrow Requisition” in “Request Forms” in “Special Instructions” and forward it with the specimen.

Bone Marrow Aspirate
Submit bone marrow aspirate in a lavender-top (EDTA) tube (found in bone marrow tray). Forward promptly at ambient temperature only.
Note:  1. Label tube with patient’s name (first and last), date of birth, date and actual time of collection, and label specimen as bone marrow aspirate.
2. Complete a “Christian Hospital Hematopathology-Bone Marrow Requisition” in “Request Forms” in “Special Instructions” and forward it with the specimen.

Bone Marrow Biopsy
Submit bone marrow biopsy in container with phosphate-buffered formalin or AZF fixative. Forward promptly at ambient temperature only.
Note:  1. Label container with patient’s name (first and last), date of birth, date and actual time of collection, and label specimen as bone marrow biopsy.
2. Complete a “Christian Hospital Hematopathology-Bone Marrow Requisition” in “Request Forms” in “Special Instructions” and forward it with the specimen.

Bone Marrow Slides
Submit 6 bone marrow slides in a slide holder.
Note:  1. Label slides/slide holder with patient’s name (first and last), date and actual time of collection, and label specimen as bone marrow smear.
2. Complete a “Christian Hospital Hematopathology-Bone Marrow Requisition” in “Request Forms” in “Special Instructions” and forward it with the specimen.
