Specimen Requirements
Plasma, serum, and 2 whole blood specimens are required.
ABO/Rh Type and Antibody Screen
Specimen Type: Blood
Container/Tube: Lavender top (EDTA)
Specimen Volume: Full tube
Specimen Minimum Volume: 10 mL
Collection Instructions: Label specimen as (blood for ABO/Rh Type and Antibody Screen).
Specimen Labeling Requirements
Christian Hospital/NW HeathCare
Christian Hospital (CH) Armband
Label CH armband section with:
1. Patient’s first and last name
2. Medical Record Number
3. Date and time drawn
4. Name/initials/number of individual labeling CH armband
Insert section into CH armband. Attach CH armband to patient. (One CH armband is used for entire visit.)
Initial Blood Bank Specimen
Label specimen with:
1. Patient’s first and last name (A printed patient label is acceptable.)
2. Medical Record Number
3. CH number (label)
4. Date and time drawn
5. Name, initials, or ID number of individual drawing specimen

Label specimen with:
1. Patient’s first and last name
2. Date of birth
3. Social Security number
4. Date and time drawn
5. Name, initials, or ID number of the individual drawing specimen
CBC
Specimen Type: Blood
Container/Tube: Lavender top (EDTA)
Specimen Volume: Full tube
Collection Instructions: Label specimen as (blood for CBC).
HBsAg, RPR, and Rubella
Specimen Type: Serum
Container/Tube: Gold top, serum gel
Specimen Volume: 3 mL
Collection Instructions:
1. Spin down unopened (vacuum intact) tube within 45 minutes of draw.
2. Label specimen as (serum for HBsAg, RPR, and rubella).
Additional Information:
1. Date of draw is required.
2. Performance has not been established using cadaver or body fluids other than serum.
3. The following specimens will not be tested:
A. Heat-treated, hemolyzed, icteric, or lipemic
B. Containing particulate matter, precipitate, RBCs, or sodium azide
